   - Child's Birth Certificate
   - Child's Immunization Records
  - Any Court Order/Divorce Decree Pertaining to Custody -

Without a court order stating otherwise, both biological parents will have equal access to the student.

These forms are required by our District.  If you are registering an Elementary student (Grades 1-5), please use the Elementary Release of Information form - If you are registering a Middle School or High School student (Grades 6-12), please use the High School Release of Information form.
